The Vendor is required to provide family centered case management (CM) to individuals and families placed in emergency housing who are receiving temporary assistance for needy families (TANF), with the goal of promoting housing stability, self-sufficiency, and access to necessary services through coordinated support and resource connection.
- Requirement:
• The firm shall provide up to 4 case managers, with at least 1 dedicated to serving recipients at the Kingston motel and 2 dedicated to serving recipients placed in other emergency housing locations.
• The county is open to an alternative 4th position that may support case management and client transportation.
• Case managers shall be based out of the temporary assistance unit and the Kingston motel, and shall travel as needed to meet with recipients at their assigned locations.
• CMs shall possess the requisite experience to build collaborative relationships, identify housing resources and potential employers, navigate and leverage resources, and connect recipients to resources effectively.
• Each CM shall also possess a bachelor’s degree or higher, with preference for a degree in social sciences, human services, family and child development, or similar coursework.
• CMs shall receive referrals regarding recipients identified by the county’s social welfare examiners, director, coordinated children’s services, and other agencies, as necessary.
• CMs shall include, but are not limited to, the following:
o Conduct the initial screenings of recipients and develop individualized service plans to address identified service needs.
o Conduct specific assessments for specialized services and identify unaddressed treatment, rehabilitation, and support needs to guide employment planning.
o The firm’s CMs shall collaborate with the county’s and employment social welfare examiners to determine the appropriate types of activities and number of hours to include in the recipient’s employment plans, and provide ongoing guidance in case decision-making and effective engagement with individuals.
o Assist recipients with complex needs in navigating state and federal benefits programs and facilitate ongoing program compliance, including targeted support for at-risk clients with a history of non-compliance.
o Connect recipients with behavioral health needs to single point of access (SPOA) and children’s single point of access (CSPOA) through referral.
o The firm’s CMs shall be active participants in SPOA and understand its functions and processes.
o Identify recipients’ treatment, rehabilitation, and support needs based on relevant information and link them to appropriate providers.
o The firm’s CMs shall monitor client progress and provide supportive services to facilitate attendance at appointments when needed, including assistance with transportation.
o Provide support to recipients experiencing crisis situations, including crisis intervention, connection to local crisis services, and same-day assistance with navigating application processes to address immediate needs, and peer navigation.
o Assist recipients with locating and retaining housing, including building positive relationships with landlords and temporary housing providers.
o Coordinate with organizations responsible for managing placement processes for newly created or soon-to-be-created affordable housing.
o Provide case management for unhoused families placed in hotels and motels.
o This includes reviewing compliance with the recipient’s individual service plan, ensuring follow-through with potential housing opportunities, and assisting with finding moving services if needed.
