The Vendor is required to provide supportive housing and case management services for include:
1. Housing management and master leasing
• Master lease operations: the provider is responsible for securing and managing scattered-site housing units under a master lease model.
• Subleasing: the vendor holds the primary lease and subleases units to participants who typically face barriers to traditional lease approval due to criminal history, behavioral health diagnosis, or lack of credit and financial barriers.
• Housing search coordination: staff must collaborate with participants to identify appropriate scattered-site apartments that meet their needs and preferences.
• Property maintenance and inspection: ensure all units meet safety standards and manage necessary repairs or cleaning as required.
2. Participant outreach, intake, and stabilization
• Referral integration: coordinate with the department and the office of public safety (ops) to process referrals from the department of prisons and other justice partners.
• Eligibility verification: target individuals with serious mental illness and coccurring substance use disorders (especially opioid use disorder) who are transitioning from local jail.
• Documentation and benefits: within the first month of a participant residing in an apartment, the provider must:
o Initiate the process for obtaining legal identification.
o Complete a licensed professional of the healing arts (LPHA) assessment.
o Initiate the SSI/SSD application process for non-able-bodied participants or make employment referrals for those who are able-bodied.
3. Evidence-based support services
• Service delivery model: implement and adhere to the following evidence-based practices:
o Housing first: prioritizing permanent housing without preconditions.
o Harm reduction: reducing the negative consequences of drug use.
o Motivational interviewing: enhancing participant motivation for positive change.
• Case management and peer support: establish a schedule of face-to-face contacts tailored to participant needs and provide on-call crisis support.
• Care coordination: assist participants in accessing and navigating social, physical, and behavioral health services to strengthen community stabilization.
4. Administrative and data requirements
• HMIS utilization: record all participant-level data in the homeless management information system (HMIS) according to HUD data quality standards.
• Reporting and collaboration: work closely with the office of public safety (ops) program analyst to facilitate data collection, reporting, and program implementation.
• Subsidy management: in the event of a participant's discharge, ensure that any remaining rental funds are used exclusively to provide housing for other current participants.
