RFP Description

The Vendor is required to provide the healthy brain program two years ago with the vision that everyone deserves the healthiest brain possible; the program focuses on key outcomes:
• Expanding community partnerships
• Enhancing prevention strategies and integrating with other chronic disease efforts
• Increasing policy action and implementation
• Improving data availability, quality, and utilization for data-informed decision-making
• Reducing stigma and bias around cognitive decline
• Building knowledge and skills among the current and future workforce
• Raising public awareness of brain health, dementia risk factors, and the importance of prevention, early detection, and diagnosis
• Promoting the use of services for people with dementia and their caregivers.
- The agency has also established the healthy brain coalition, bringing together stakeholders from across state to develop a state plan to promote brain health, risk factors for dementia, benefits of early detection and diagnosis, and support for people living with dementia and their caregivers.
• Increase public knowledge about brain health, risk factors for dementia and benefits of early detection and diagnosis
• Engage diverse audiences to develop culturally responsive messaging about brain health, cognitive decline, healthy aging, and caregiving.
• Disseminate culturally responsive messaging to encourage conversations about brain health, cognitive decline, healthy aging, and caregiving.
• Engage with communities, especially those at highest risk, about risk factors for dementia and how people living with dementia can best thrive in their communities.
• Embed cognitive decline risk factors into evidence-informed health promotion and chronic disease prevention awareness and education campaigns.
• Partner with educational systems (K-12 and postsecondary) to include brain health and dementia in their curricula. Increase public knowledge and use of services for people living with dementia and their caregivers
• Enhance communication with people living with dementia, their families, and caregivers about how to access services, care, and social supports.
• Ensure caregivers have information, tools and resources about their vital role and ways to maintain their own health and well-being.
• General Public: The campaign seeks to raise awareness among people of all ages in state about brain health, dementia risk factors, benefits of early detection and diagnosis.
• Family members and caregivers: information and resources are provided for people who care for loved ones living with department, offering support, guidance, and connections to available services.
• People diagnosed with dementia: individuals who have received a diagnosis impacting brain health and may require guidance, education, and resources to navigate their next steps. give them hope for the best quality of life possible throughout each step of their journey.
• Healthcare providers: healthcare professionals are equipped with knowledge and tools to promote brain health, recognize early signs of cognitive decline, and guide patients and their caregivers toward appropriate services.
• Older adults and people at higher risk: the campaign encourages people at higher risk for cognitive decline, including older adults, to adopt proactive health measures, and understand the importance of early detection and risk reduction practices.
• Community leaders and organizations: local leaders and community-based organizations are engaged to strengthen partnerships and foster a supportive environment for brain health initiatives across the state, reducing stigma and increasing dementia friendly communities.
• Workforce development: current and future healthcare and social service professionals are educated on brain health, risk reduction strategies, and caregiving best practices to build a knowledgeable and skilled workforce.
• People in rural and underserved communities: outreach efforts focus on people in rural and underserved communities across state, addressing potential barriers to accessing brain health resources and services.
