RFP Description

The vendor required to provide intensive case management (ICM) services to individuals in the community connections supportive housing (CCSH) program who are unhoused, homeless, or precariously housed, those with mental illness, co-occurring disorders, and disabilities, or whose lack of unmet social needs has resulted in the involvement of the criminal justice system.
- Intensive case management (ICM) and wrap around services is to help clients reduce the use of emergency services, maintain permanent housing, and access additional needed supports necessary to be successful in permanent housing.
-  For those released from incarceration, this program will aid in the reduction of recidivism, increase compliance with court directives, maintain permanent housing, and increase access to additional supports to be successful in permanent housing.
- Program description:
•    Rental assistance is administered through rental vouchers paid directly to landlords on behalf of eligible clients. 
•    Parid with comprehensive wrap-around services, including ICM, behavioral health support, and l linkage to community resources, the program is designed to help program clients secure and maintain stable housing, reduce reliance on emergency services, and enhance overall well-being. 
•    Justice involved individuals, these supports also promote the reduction of recidivism, improved compliance with court or supervision directives, and increased access to the services needed to achieve long-term housing stability. 
•    Community connections is a harm reduction model that does not require initial abstinence from substance use but does require interest and willingness to reduce harm associated with substance use. 
•    The harm reduction model is to maximize access to and minimize loss of housing, while considering the rights of the individual. 
•    This means that while residents will not be required to be abstinent from substance use upon entry into the program, they will be expected to connect with community services and supports, seek and secure employment, and or increase income, and actively participate in activities provided through case management services. 
•    Continued participation in the program is determined by program adherence.
- Service delivery:
•    Case management staff will provide intensive case management (ICM) services. 
•    These services will help individuals navigate and negotiate a complex service system and obtain access to treatment and services that support and sustain their recovery, based on goals generated in the client's ICM service plan. 
•    ICM emphasizes the importance of a small caseload, more frequent contact, and potentially a longer duration of contact. 
•    ICM provides an opportunity to expand the network of services for individuals by formally navigating through a continuum of interagency community resources to help clients attain necessary assistance.
•    ICM emphasizes the importance of a small caseload (a maximum of 1:10 to 12 ratios at high intensity services), more frequent contact (greater than 1 contact and week), and potentially a longer duration of contact (greater than six months).
•    ICM provides an opportunity to expand the network of services for individuals by formally navigating through a continuum of interagency community resources to help clients attain necessary assistance. 
•    Comprehensive ICM services help identify individual needs and resources, including provider linkages and potential gaps in services for clients to meet social, health, welfare, housing, vocational, and other needs to attain and sustain recovery.
•    ICM is also a program of assistance to help clients meet their basic needs, such as: accompanying the client to appointments to provide introductions and support; arranging for clients to receive individual therapy and group therapy; ensuring the essential needs of the client are being met, including access to food, clothing, housing, health care, including medication assisted treatment (mat), and social supports. 
•    A client service plan of tracked goals and activities, developed between the case manager and client, is a fundamental tool that is based on the client's needs assessment. 
•    The program may create a step-down level of client assistance based on defined benchmarks, such as demonstrating progress through achieving measurable goals, and requiring less than one contact and week.
•    ICM services should be person-centered and structured around the individual’s strengths and assets. 
•    This means putting people as equal partners with the clinical team in planning, developing, and monitoring care to make sure their needs are met. 
•    The client is at the center of decisions, working alongside professionals to achieve the best outcome. 
•    A strengths-based approach allows case managers to acknowledge that every individual has their own set of strengths and abilities to rely on to overcome problems.
•    Services shall include an array of wrap-around services as appropriate to client needs identified in the individual service plan, to be provided by the applicant or in partnership with other agencies. 
•    Intensive case management services should include, but not be limited to, individual and group therapeutic treatment; life skills training; vocational assistance where appropriate; access to medical and psychiatric health care, as well as medication when appropriate; access to public benefits; and support in maintaining stable housing through the duration of the funding period.
- Requirement
•    CCSH clients participating in case management have access to intensive case management (ICM) services throughout their engagement, housing transition, stabilization, and community integration. 
•    Maintain a case load consistent with ICM services and document all client interactions in a act-compliant electronic health record (EHR) and electronic medical records (EMR) system within forty-eight (48) hours. 
•    The contractor will schedule a clinical assessment for all clients referred to them by the program assessments will be scheduled annually or more frequently if needed. 
•    The contractor shall strongly encourage participation in case management services without making participation a condition for ongoing tenancy. 
•    The contractor will ensure that case managers, clinical staff, and program leadership will attend monthly multidisciplinary meetings for case staffing, data review, and service coordination.
