The Vendor is required to provide case management services and care coordination for county residents under the following programs: alternative care (AC), brain injury (BI) waiver, community access for disability inclusion (CADI) waiver, community alternative care (CAC) waiver, developmental disability (DD) waiver, elderly waiver (EW), essential community supports (ECS) rule 185 case management and vulnerable adults/developmental disability targeted case management (VA/DD TCM).
- Services are voluntary and through person-centered practices.
- This includes an increased need for case management services.
- Additionally, several current providers are experiencing challenges with staff turnover and budget constraints.
- The County currently has a wait list for case management assignment.
- Providers will be expected to accept referrals weekly, depending on county need.
- Individuals will receive case management services that support and assist them in locating and gaining access to necessary waiver services as identified in the state assessment, as well as medical, social, educational, employment or other needed services regardless of the funding source.
- Provide culturally responsive programming to ensure effective, equitable, comprehensive, and respectful quality care services that are responsive to individuals within a specific population’s values, beliefs, practices, health literacy, preferred language, and other communication needs.
- Providing culturally responsive programming is designed to address the unique needs of individuals who share a common language or racial, ethnic, or social background.
- Case managers are instrumental in assisting with finding and putting necessary services in place to allow residents to live in the community as safely and independently as possible.
