The Vendor is required to provide for strategic planning services.
- Population health and wellbeing, including mortality measures, prevalence of chronic physical and mental health conditions, and life satisfaction measures;
- Community conditions and drivers of health, including healthcare infrastructure and affordability, housing and transportation infrastructure and affordability, environmental and climate health, social and economic factors, and civic and community resources;
- Structural determinants, including healthcare institutional policies, health and social policies, governance, cultural factors, social norms, and stigma; and
- Power dynamics, including the history of systemic racism and contemporary power structures in Central state, indicators of structural racism, and community power and assets.
- Plan and facilitate community engagement efforts with representation from each of the catchment area counties to garner community input and qualitative insights to drive the agency thematic narrative and support quantitative findings.
