Comprehensive Street Outreach Services

The Vendor is required to provide outreach programs that focused on making initial “contacts” to those incorporating more robust services such as housing navigation and clinical supports.
- Services and outcomes
• New or expanded levels of outreach services, facilitating a more comprehensive approach than currently available resources
• Transportation will be provided to participants seeking to access shelter, housing, or other services without their own access to transportation
• Program will facilitate increased access to services for persons experiencing substance abuse and mental illness
• Project will facilitate access to the local housing triage system as well as other forms of housing (income-based, market rentals, etc.)
• Explain the project’s plan for ongoing review and analysis of performance and outcome.
