The Vendor is required to provide to street outreach, engagement and substance abuse services.
- Responsible for providing enhanced homeless street outreach, engagement, case management, and referral services to primarily benefit individuals experiencing homelessness within the City.
- Outreach teams shall meet individuals experiencing homelessness where they are located and provide supportive wraparound services designed to connect individuals to shelter, treatment, and long-term housing opportunities within the City
- Conduct eligibility screening and provide referrals to appropriate services including, but not limited to:
• Emergency shelter programs
• Recuperative care programs
• Detoxification and substance use treatment programs
• Crisis stabilization units
• Behavioral health services
• Medical health facilities
• County behavioral health and social service programs
- Outreach teams shall provide field-based case management services for individuals who are unable or unwilling to immediately enter shelter or treatment.
- Shall utilize best practice engagement models including:
• Progressive engagement
• Motivational interviewing
• Trauma-informed care
• Strength-based service delivery
- Program components:
• Street Outreach and Engagement: Field-based outreach services focused on engagement, rapport building, and connecting individuals experiencing homelessness to services and housing resources.
• Housing Navigation: Assistance with accessing shelter, housing programs, and supportive housing opportunities including document readiness and housing placement support.
- Substance Use Disorder Services: The Contractor shall incorporate a substance use disorder outreach component that includes:
• Screening and assessment of substance use disorders
• Brief interventions and motivational engagement
• Referrals to detoxification and treatment programs
• Assistance accessing residential or outpatient treatment
• Coordination with treatment providers for intake and placement
- Transportation Services: Transportation assistance shall be provided to support client access to shelter, detox programs, treatment facilities, medical appointments, and other services.
- Data Collection and Reporting: All outreach activities and service outcomes must be documented and reported in accordance with City and Continuum of Care data requirements.
