The Vendor is required to provide peer-led outreach and engagement services.
- Provide a multidisciplinary, field-based program designed to engage individuals experiencing homelessness who face complex barriers related to mental health conditions, substance use, chronic medical needs, and justice system involvement.
- Peer Outreach Specialist Responsibilities
• Conduct field-based outreach and engagement with individuals experiencing homelessness, including those with a history of justice system involvement
• Utilize lived experience to build rapport, reduce stigma, and establish trust with participants who are disengaged from traditional systems of care
• Support participant engagement and retention in agency services
• Assist with housing navigation, including:
• Accompaniment to housing appointments
• Support with housing applications and documentation
• Coordination with housing providers and case managers
• Provide systems navigation and advocacy, including linkages to:
• Behavioral health and substance use treatment
• Primary and specialty medical care
• Benefits enrollment (e.g., Medi-Cal, SSI/SSDI, CalFresh)
• Legal, reentry, and re-stabilization services
• Model and apply trauma-informed, harm reduction, and culturally responsive practices
• Collaborate with agency staff to support care planning, follow-up, and warm handoffs
• Participate in required trainings, meetings, and data reporting activities
- Flexible Support Resources:
• Emergency transportation (e.g., transit passes, rideshare, gas cards)
• Food assistance and basic needs support
• Replacement of vital documents and identification (e.g., birth certificates, state ID)
• Other short-term supports that directly facilitate access to housing, healthcare, or benefits.
