- Project work plan and stakeholder engagement
• Project work plan: approach for identifying tasks, milestones, dependencies, responsible personnel, stakeholder activities, draft-review periods, and completion dates.
• Stakeholder engagement: approach for engaging rural and tribal healthcare providers, critical access hospitals, federally qualified health centers, referral centers, specialty providers, behavioral health providers, local public health units, telehealth facilitators, patients, caregivers, payers, technology partners and other stakeholders identified by state.
• Engagement methods: proposed use of interviews, listening sessions, surveys, document review, data requests, or other methods to obtain consistent and representative information.
• Required deliverable: a project work plan, stakeholder-engagement plan, data collection plan, and project schedule.
- Current state and needs assessment
• Existing resources: current telehealth services, platforms, equipment, connectivity, referral relationships, staffing, workflows, utilization, investments, and technical support.
• Access needs: services associated with limited availability, long wait times, substantial travel, or out-of-state referral.
• Patient and provider barriers: geographic, transportation, broadband, device, digital-literacy, workforce, scheduling, privacy, accessibility, reimbursement, and operational barriers.
• Priority use cases: services suitable for direct-to-patient telehealth, facilitated telehealth, provider-to-provider consultation, remote patient monitoring, or other virtual-care models.
• Required deliverable: a current-state and needs assessment identifying existing assets, gaps, priority services, populations, providers, and geographic areas.
- Governance, reimbursement, and sustainability
• Governance: oversight structure, decision authority, participation expectations, accountability, performance oversight, and dispute resolution.
• Regulatory considerations: licensure, credentialing, privileging, consent, documentation, prescribing, privacy, provider and patient location, and tribal participation.
• Reimbursement: Medicare, Medicaid, commercial insurance, remote patient monitoring, teleconsultation, facility fees, and other applicable reimbursement considerations.
• Sustainability: startup and recurring costs, utilization assumptions, revenue, cost sharing, replacement costs, funding alternatives, and financial risks.
• Required deliverable: a governance and sustainability report containing recommended roles, operating responsibilities, reimbursement strategy, financial assumptions, and long-term Sustainability recommendations.
