The vendor is required to provide administer a customized wellness platform for agency employees, spouses and children.
- Current activity options include health risk assessment, biometric screenings, primary care visit, preventive care screenings and exams, on-site programs, wellness challenges, community events, volunteering, fitness classes, and health log tracking.
- Activity registration and completion are currently tracked in a database and wellness platform.
- The platform should coordinate with various internal and external vendors of metro to incorporate their data and programs via direct feed for seamless program delivery to remove any manual uploads of data on the metro wellness team.
- The platform also needs to be available for resale to external clients of sky well, under the sky well name.

• 2,881 agency spouses
• 8,578 employees from sky well clients"
- The agency system’s long-term strategy is to use data from the platform and any integrated vendor programs to develop and implement plans and programs that:
• Are based on current and future cost drivers, using agency -specific data (where available) to support initiatives and measure program effectiveness and VOI.
• Address a broad view of health and well-being issues.
• Focus on increased engagement that leads to long-term, sustainable behavior change.
• Have a total population health focus identifying and addressing health disparities and social determinants or drivers of health to close gaps in care, improve condition improvement plan adherence and lower medical plan costs."

• Educational content (i.e., integrated wellness-related workshops and webinars with the ability to customize and create additional learning opportunities)

• Wellness tools (i.e., health trackers, physical activity trackers, nutrition trackers, meal plans, healthy recipes)
• Direct connectivity to most activity trackers and fitness applications
• Marketing templates and photo library that can be customizable communications
• Free mobile application for members to download to any device that provides all the same components that the wellness platform provides
• Event and program registration for fitness classes and webinars
2. Individualized action plans and on-line programs
• Coaching based tobacco cessation program may be offered by the awarded proposer or the agency system may elect to use their in-house resources.
• Individual and team-based challenges administered as a collaboration between the agency system and proposer.
• Disease management programs administered by the agency system, system’s health plan administrator or potential products available from the awarded proposer, or a third-party proposer.
• Health coaching programs may be offered by the awarded proposer and/or agency.

3. Real time reporting
• A dashboard easily accessible for client to see real time reports showing completion of health assessment, biometric screenings, challenge participation other incentive program requirements, etc. provide on-going data analysis, interpretation, forecasting and benchmarking.
• Agency also needs to have access to individualized participant data 24/7 for the medical director and biostatistician to review.
4. Ability to connect with multiple outside data sources
• The platform needs to allow for verified information to be uploaded into the platform for automatic incentive points to post to participants accounts timely alleviating the manual data entry on the current agency wellness team administering the program.
5. Integration with outside vendors
• Vendor must have the ability to interface with multiple third-party vendors for results reporting.
6. Flexible and customizable platform
• Agency will want to customize options available to our users and work with the vendor to create special features specific for our needs and client needs.
6. Advisory services
• Review current wellness program offerings and recommend enhancements to enhance user engagement and VOI while ensuring compliance with wellness program rules.
• Provide and recommend innovative ideas, new products, programs and services based on industry best practices.
• Provide presentations to senior executives, or other agency representatives as requested.
• Support the creation of case studies, white papers and content creation for national worksite wellness award applications.
7. Marketing and communication support
• Provide turn-key modifiable marketing templates and photo library to assist agency with communications of new programs or changes to existing programs.
8. Customer support service
• Provide call center for technical and program support to agency employees, dependents and outside clients participating in the wellness program.
10. Mobile app
• Wellness platform needs to have a companion app that participants can download with the ability to receive alerts and reminders of upcoming events and specific and tailored message alerting them to take action on a health and well-being measure (ex. remember to take your medication today, log your steps in the challenge, log your fruits and veggies, reach out to your accountability partner, etc.).
11. Health coaching
• The health improvement and coaching programs should include physical activity, nutrition and other wellness programs.
• Supportive wellness coaching for participating members upon request either telephonically, secured electronic messaging and texting, or virtually via secured face – to- face web meetings.

• Provide a brief history of your firm including size, industry experience, volume of business, locations, number of years in business and business philosophy
