The vendor required to provide confidential shredding services to complete destruction of confidential and sensitive documents, including but not limited to paper, paper clips, binder clips, sticky notes, glossy paper, no carbon required (NCR) paper, fax paper, hanging file folders, binders, floppy disks, microfilm/fiche, video/audio tapes, cd’s, and fax rolls.
- Liaison or designee will notify the contractor at least forty-eight (48) hours in advance to schedule shredding services.
- Provide at least twenty-four (24) hours advance written notice to the agency contract liaison or designee if they cannot make a scheduled service visit.
- The delay in service is due to breakdown of the mobile shredding machine, the contractor shall notify the agency contract liaison or designee immediately.
- Receipts
• Provide a “certificate of destruction” as well as a receipt to the agency contract liaison or designee at the completion of the service visit.
• The receipt shall include the location of service, agency contract liaison or designee name and phone number, number of pounds destroyed, date of service, the authorized contractor representative(s), and agency contract liaison or designee witness’ signatures as well as the agreement number.
- Shredding equipment shall be maintained in accordance with all applicable federal, state, and local codes and regulations, including the authority having jurisdiction.
